The Town of Edgewood and Santa Fe County bring a fun-filled summer recreation program to Edgewood in June and July for youth ages 5-11.

Cost per camper: $50/week -or- $400/full 8 wks. Limited space, available to Santa Fe County and Town of Edgewood residents.

RELEASE OF LIABILITY - As a parent/guardian of the aforementioned child, I hereby give my approval and/or permission for my child to participate in the YES! Summer Recreation Program sponsored by Town of Edgewood. I know that participation in individual and group indoor and outdoor activities may result in injury. I assume all risks and dangers to the participant arising from participating in, watching, or attending YES! Summer Receration Program. I release Town of Edgewood (NM), Santa Fe County (NM), Moriarty-Edgewood School District, and the YES! Summer Recreation Program leaders, assistants, their prospective administrators, directors, agents, officers, officials, members, consultants, volunteers, and employees, other participants, and owners and lessors of the premises on which the activity takes place, from all liability, costs and damages which might arise from participation in the YES! Summer Recreation Program. I further provide my consent for an adult member of YES! Summer Recreation Program to seek emergency treatment for my child/ward if necessary. I agree to accept financial responsibility for the costs related to this emergency treatment as well as any other medical treatment resulting from participation in YES! Summer Recreation Program.

The Town of Edgewood Parks & Recreation department is committed to providing campers with opportunities for fun activities and friendship. In order to maintain a positive and safe environment, all campers are expected to participate and follow this YES! code of conduct:

Be Prepared By:

Wearing appropriate clothing and shoes to camp each day.

Coming to camp ready for fun and new friendships.

Being ready to try new experiences and activities.

Be Respectful By:

Treating all campers and staff with courtesy and kindness.

Including all campers in activities with a smile and friendship.

Listening to staff directions and to other campers’ ideas and opinions.

Treating everyone’s belongings with care and thoughtfulness.

Be Safe By:

Staying with your group and your counselors at all times.

Following the rules and being kind to others at all times,

Informing camp staff if you have difficulty with another camper, activity, or situation.

I understand the YES! experience should be fun and friendly for ALL campers, and that if my behavior or actions do not follow the code of conduct that these consequences may result:

Verbal counseling/redirection from camp staff

Timeout from scheduled activities

Camp Report (Parent/Guardian Notification)

Parent/Guardian conference with Program Director or Town Staff member

Suspension or dismissal from camp

Consent
